Morphological classification of ventricular septal defect by color doppler and 2-dimensional echocardiography



Abstract
In order to determine the relative incidence of the various types of ventricular septal defect and to evaluate their complications or secondary changes, the echocardiographic findings of 381 patients(age 1 month to 15 years) with isolated
ventricular
septal defect who had been diagnosed by color Doppler ultrasound system from October 1988 to July 1992 were reviewed.
@ES The results were as follows:
@EN 1) Among the 947 patients with congenital heart disease, the incidence of isolated ventricular septal defect was 40.2%.
2) Among the 381 patients with ventricular septal defect, 206 patients were boys and 175 patients were girls. (Male to female ratio was 1.2:1). The age distribution was 5 years or younger in 289 cases(75.9%), between 6 and 10 years in 71
cases(18.6%),
and between 11 and 15 years in 21 cases(5.5%).
3) Of 381 cases 284 cases974.5%) were perimembranous, 86 cases(22.5%) infundibular[60 cases(15.7%) subarterial infundibular, 26 cases(6.8%) muscular infundibular], and 11 cases(2.9%) muscular type. Of 11 muscular types, 9 were single and 2 were
multiple defects.
4) So called septal aneurysm aneurysm was noted in 38.0% and tricuspid regurgitation in 13.7% of the patients with perimembranous ventricular septal defect.
5) The aortic valve prolapse ws noted in 47.7% and aortic regurgitation in 19.8% of the patients with infundibular ventricular septal defect.
Thus, this observation reassures the previous reports of the higher incidence of infundibular ventricular septal defect, especially subarterial infundibular type with the frequent association of the aortic valve prolapse and aortic regurgitation,
and
the lower incidence of muscular ventricular septal defect in Orientals including Koreans compared with Occidentals, implying differences.
